Palliative & End-of-Life Care

Palliative care is medical care focused on improving the quality of life for people living with serious, chronic, or life-limiting illnesses. It aims to relieve symptoms such as pain, breathlessness, nausea, fatigue, and anxiety, while also providing emotional, psychological, and spiritual support.

End-of-life care refers to care provided when a person is approaching the final phase of life, typically when treatment is no longer curative and the focus shifts entirely to comfort, dignity, and quality of remaining life. It often involves managing distressing symptoms, supporting the patient’s wishes (including advance care planning), and providing guidance and support to family members.

Geriatric & Age-Related Conditions

Geriatric and age-related conditions refer to health issues that commonly affect older adults due to the natural ageing process, long-term medical conditions, or a combination of both. These conditions often involve multiple body systems and may impact physical health, memory, mobility, independence, and overall quality of life.

Common examples include dementia and memory disorders, osteoporosis, arthritis, frailty, balance problems and falls, hearing or vision impairment, urinary incontinence, and chronic diseases such as heart disease or diabetes. Older adults may also experience increased sensitivity to medications and a higher risk of complications from illness.

Geriatric care focuses on comprehensive assessment and coordinated management, aiming to maintain independence, improve daily functioning, prevent hospitalisation, and enhance quality of life. Care plans are often tailored to the individual’s medical needs, social circumstances, and personal goals.

Disability & Neurological Conditions

Disability and neurological conditions refer to physical, cognitive, or functional impairments that may result from disorders affecting the brain, spinal cord, nerves, or muscles. These conditions can be present from birth (congenital) or develop later in life due to illness, injury, or degenerative disease. They may impact mobility, coordination, speech, memory, behaviour, sensation, or the ability to perform daily activities independently.

Common neurological conditions include stroke, epilepsy, multiple sclerosis, Parkinson’s disease, motor neurone disease, cerebral palsy, and traumatic brain or spinal injuries. Some individuals may also live with developmental or intellectual disabilities that affect learning, communication, or social functioning.

Management focuses on improving quality of life, maintaining independence, preventing complications, and supporting both the individual and their caregivers. Care may involve medication management, rehabilitation, physiotherapy, occupational therapy, assistive devices, and coordinated long-term medical support tailored to the person’s specific needs and goals.

Mental Health & Neurodivergent Disorder 

Mental health conditions are disorders that affect a person’s mood, thinking, behaviour, and emotional wellbeing. They can range from mild to severe and may impact daily functioning, relationships, work, and overall quality of life. Common examples include anxiety disorders, depression, bipolar disorder, post-traumatic stress disorder (PTSD), and schizophrenia. These conditions are treatable and often managed through a combination of psychological therapy, medication, lifestyle strategies, and ongoing medical support.

Neurodivergent disorders (or neurodevelopmental differences) refer to natural variations in how the brain processes information, learns, communicates, and interacts with the world. Rather than being viewed solely as illnesses, neurodivergence recognises that some individuals think and function differently from what is considered typical. Examples include autism spectrum disorder (ASD), attention-deficit/hyperactivity disorder (ADHD), dyslexia, and Tourette syndrome.
