Our vision is simple: every homebound person—whether living in a private residence or aged care facility – deserves timely, coordinated, and person-centred primary care. By removing transportation barriers and prioritising continuity of care, we strengthen patient-provider relationships, improve health outcomes, and help prevent unnecessary emergency presentations and hospitalisations.

Who We Support
Cornerstone Care provides medical services to individuals who are homebound and do not currently have a regular GP, or who are seeking a regular GP. A person is considered homebound if leaving their home is either impossible or would cause significant medical or psychological difficulties. There is no age restriction to access this service. However, all referrals will undergo a triage process to assess suitability and ensure the service is appropriate for the individual’s needs.

Going through palliative care- whether you are the one receiving it or supporting someone who is- can be incredibly challenging.
Chronic Medical Conditions
COPD, heart failure or any medical conditions that cause individuals to be homebound.
Geriatric & Age-Related Conditions
Dementia, poor mobility, high falls risk, and visual impairment that make it unsafe for individuals to leave their home.
Disability & Neurological Conditions
Stroke, motor neuron disease, or any neurological disorders that render mobility difficult.
Mental Health & Neurodivergent disorder
Conditions where leaving the house cause significant psychological distress eg. ASD, OCD, intellectual impairment etc.
Post surgery care
Being rendered homebound post surgery due to poor mobility, pain and activity restriction.

Cornerstone Care is established to fill in a significant gap that the current primary health care system is presenting. Homebound individuals could feel that they are being marginalised and are not able to receive the healthcare that they deserve. Cornerstone Care addresses this gap by providing comprehensive home visits and telehealth services designed to improve access, enhance outcomes, and promote equitable continuity of care.
The primary objectives are focused on the following:
Improved access to care
We aim to eliminate transport and mobility barriers, ensuring homebound individuals receive timely and consistent medical attention.
Enhanced quality of care and outcomes
Homebound individual can and should still receive continuity of care including preventative health care, diagnosis, treatment, management, coordination of comprehensive care and referral to specialists and allied health services.
Increased patient and caregiver satisfaction
We aim to foster strong patient-provider relationship by having a consistent point of contact with the same healthcare provider team, hence improving patient rapport and trust.
Reduced healthcare costs and burden
We aim to prevent unnecessary or avoidable emergency room visits and hospitalisations.
How It Works
Our simple, patient-focused process ensures you receive the right care at the right time, with minimal stress for you and your family.
Referral
Self-refer or a family member can refer their loved one. Specialists, other GPs or relevant healthcare providers can make referrals as well.
Triage
Cornerstone Care will triage the referral based on suitability, urgency and availability of service.
Consultation - home visit
Home visits will be carried out according to the agreed time with the individual.
Subsequent care
Subsequent care can be provided through home visits or telehealth, on a regular schedule or as needed.
