Receive Aged Care Visit
Individuals can either self-refer or be referred by family members, specialist, or any healthcare providers. Cornerstone Care GP will then triage the referral and assess suitability of the individual to receive aged care visit. Suitable candidate will be contacted and offered appointment for aged care visit.
