This model improves access to essential services, strengthens continuity of care and patient–doctor relationships, and helps reduce avoidable hospital visits. Referrals can be made by patients, family members, or healthcare providers, and Cornerstone Care GP assesses each referral to determine suitability before arranging a home visit appointment.
